Family-Centred Disability Support

Support That Includes Everyone Because You Matter

At Horizons of Hope, we believe that when one person needs care, the whole family deserves support. Too often, providers only focus on the person with the NDIS plan, leaving families to figure out the rest. That’s where we do things differently. Our approach is built around a family centred model, because true care doesn’t stop with the participant. We walk alongside everyone involved; participants, parents, carers, siblings, grandparents, and extended family, offering wrap-around support with compassion, strategies, and real help, in the moments that matter most.

What We Offer:

In-home support to ease the daily load and bring calm into your routine

A compassionate second set of hands for parents and caregivers juggling too much

Emotional and behavioural regulation strategies to help participants and families thrive together

Flexible, family-first solutions designed to bring connection, not just coping
